Significance of fatty tissue hormones in the progression and expansion of metabolic disorders in patients with post-infarction cardiosclerosis

The goal of research is to estimate the significance of fatty tissues' hormones in the progression and expansion of metabolic disorders on patients with post-infarction cardiosclerosis, diabetes mellitus type 2 and fatty degeneration.
A complete physical examination of 295 patients with post-infarction cardiosclerosis was carried out. The patients have been receiving medical treatment in the cardiologic and infraction departments of Kharkiv`s city clinic №27 CHPI which is a basic medical treatment facility of the Department of Internal Medicine №2, Clinical Immunology and Allergology of Kharkiv National Medical University of MOH of Ukraine. All patients with post-infarction cardiosclerosis were divided by groups depending on presence of DM type 2 (first group (n=68)), fatty degeneration (second group (n=76)) and their combined content (third group (n=71)). Comparison group was consisted of 80 patients with post-infarction cardiosclerosis without diabetes and fatty degeneration. The control group included 35 apparently healthy persons. Groups were comparable by age and gender. Research did not include patients with severe comorbidity of respiratory apparatus, organs of the digestive system, kidneys and persons with oncology diseases.

Patients with post-infarction cardiosclerosis, DM type 2 and fatty degeneration having been determined truly higher value of vaspin and lower value of omentyn against patients with post-infarction cardiosclerosis and control group (Table 1). Level of vaspin for patients of main group was 831.883±3.789 pg/ml and was almost 2 and 4.5 times more than those for patients of comparison group and control group (р<0.001). Level of omentyn, at opposite, for patients of main group was 302.005±2.899 ng/ml and was lower by 1.7 and 1.9 times than for patients with post-infarction cardiosclerosis and control group (р<0.05). Thus, the imbalance of aditocypokines for patients with post-infarction cardiosclerosis is associated with progression of DM type 2 and fatty degeneration.We carried out a comparative analysis of the state of adipocytokinetic metabolism between major test groups showing that multimorbidity of post-infarction cardiosclerosis, DM type 2 and fatty degeneration leads to more expressive changes of fatty tissue hormones. So, the level of vaspin for patients with post-infarction cardiosclerosis, diabetes and fatty degeneration was 831.883±3.789 pg/ml and was more by 25.01 % in comparison with patients suffered from DM type 2 only; by 36.11 % in comparison with patients suffered from fatty degeneration only and by 49.24 % in comparison with those for patients having no metabolic disorders (Р<0.001). Level of omentyn in multimorbid patients was equal to 302.005±2.899 ng/ml which was by 28.22 %, 32.20 % and 41.88 % lower in comparison with patients having post-infarction cardiosclerosis and DM type 2, post-infarction cardiosclerosis and fatty degeneration and post-infarction cardiosclerosis correspondingly (Р<0.001).

Studying of fatty tissue hormones levels of patients who have post-infarction cardiosclerosis, DM type 2 and fatty degeneration has shown an increase of vaspin level on the background of increasing of IR degree from I to III tertile (811.436±2.258 pg/ml; 829.375±2.347 pg/ml and 851.281±2.213 pg/ml correspondingly) and decreasing of omentyn level (316.419±2.284 ng/ml; 303.589±2.367 ng/ml and 276.637±2.539 ng/ml correspondingly) (р<0.05).
Patients with post-infarction cardiosclerosis, DM type 2 and fatty degeneration of I stage has vaspin level of 806.132±3.261 pg/ml; patients with post-infarction cardiosclerosis, DM type 2 and fatty degeneration of II stage has vaspin level of 821.194±3.347 pg/ml and patients with post-infarction cardiosclerosis, DM type 2 and fatty degeneration of III stage has vaspin level of 843.194±3.273 pg/ml (р<0.05). Level of omentyn for patients with post-infarction cardiosclerosis, DM type 2 and fatty degeneration depending on ІМТ was distributed as following: 311.482±2.671 ng/ml, 296.375±2.583 ng/ml and 268.121±2.734 ng/ml for patients with fatty degeneration of I, II and III stage correspondingly (р<0.05).
